Observe failures without hiding uncertainty
Use unknown when no measurement is available. Keep enough evidence to investigate without private content. Before retrying, check whether the earlier request might already have changed data.

Count errors over all requests and keep unknown outcomes separate. Logs need event codes and status, not ticket content or login credentials.

Edit only summarizeEvents in model.mjs, run operate tests and write incident-note.md for the four-event sample. Preserve unknown, empty rate=null and exclude private payloads. State unknowns; do not claim recovery without evidence.
